Description of the Service This document certifies the surviving family members’ information, where the head of the family is deceased. It is used for availing the benefits under the Apathbhandu Scheme / Chief Ministers’ Relief Fund/ Housing, Insurance Claims, Succession of Financial Assets, and updating of records of properties and other details. Name of Applicant with Aadhar Number b. Postal Address / mailing address c. Email id d. Father / Husband Name with Aadhar number e. Mother Name with Aadhar Number 2. A Notarized Affidavit containing Name, Age, and Relationship of the Applicant with the deceased person. 3.
