Family Member Certificate process
- Description of the Service
This document certifies the surviving family members’ information, where the head of the family is deceased. It is used for availing the benefits under the Apathbhandu Scheme / Chief Ministers’ Relief Fund/ Housing, Insurance Claims, Succession of Financial Assets, and updating of records of properties and other details.

15 days - Documents Required
- 1. Application Form for Family Members Certificate including:
- a. Name of Applicant with Aadhar Number
- b. Postal Address / mailing address
- c. Email id
- d. Father / Husband Name with Aadhar number
- e. Mother Name with Aadhar Number
- 2. A Notarized Affidavit containing Name, Age, and Relationship of the Applicant with the deceased person.
- 3. Mandatory any one of the Documents (Ration Card/Voter ID Card/Passport/Bank Passbook/Aadhar etc.) which indicates the relationship of applicant with deceased person.
- 4. Death Certificate/FIR of the deceased head of the family.

- Should be a citizen of Andhra Pradesh
- Household should be mapped in the database
- Immediate Appellate Authority
- Mandal Revenue Officer (MRO)
